(722) Angiotensin II Use in Cardiogenic Shock Patients Supported by Temporary Mechanical Circulatory Support

چکیده
PurposeThe use of Angiotensin II (ANGII), the most potent endogenous vasopressor, has not been studied in patients with cardiogenic shock on temporary mechanical circulatory support (tMCS). We aimed to evaluate hemodynamic response ANGII adult tMCS and further describe patient characteristics its efficacy.MethodsCardiogenic who were supported by (IABP, Impella, VA-ECMO) received included study. Patients requiring durable MCS or right ventricular excluded. Clinical, pharmacological, hemodynamic, echocardiographic obtained at time initiation ANGII. A positive was considered if mean arterial pressure increased 10mmHg there a decrease NE equivalent dose, 30 minutes after initiation.ResultsForty met inclusion criteria, clinical are summarized Table 1. Mean age 55.7±16.8 years, 72.5% male. Twenty-three (57.5%) ECMO, 12 (30%) impella, 11 (27.5%) IABP initiation. In addition, 18 (45%) renal replacement therapy. The SOFA score 9.5±3.3 ANG Twenty-six (65%) categorized as responders their shown table1. There no significance between non-responders. mortality rate 90 days for 71.4% 69.2% (p=1) non-responders responders, respectively.ConclusionIn tMCS, administration associated significant other vasopressor medications 65% patients. Response day survival. efficacy. Cardiogenic Forty respectively.

Mechanical circulatory support in acute cardiogenic shock
Cardiogenic shock complicates about 5% to 8% of all admissions for acute myocardial infarction, and despite advancement in treatment over the past 50 years, mortality remains unacceptably high. Management with vasoactive agents after revascularization can have its limitations and thus mechanical circulatory support is often initiated. Intra-aortic balloon pumps (IABPs) are the devices most comm...
متن کاملPercutaneous Mechanical Circulatory Support Devices in Cardiogenic Shock.
Despite a high rate of early revascularization and use of intra-aortic balloon pump counterpulsation therapy, the prognosis of patients with cardiogenic shock has remained poor. In the hopes of improving outcomes, clinicians are increasingly turning to percutaneous left and right mechanical circulatory support devices. Until recently, the evidence base for these devices had consisted only of ob...

BACKGROUND Biventricular failure is associated with high in-hospital mortality. Limited data regarding the efficacy of biventricular Impella axial flow catheters (BiPella) support for biventricular failure exist. The aim of this study was to explore the clinical utility of percutaneously delivered BiPella as a novel acute mechanical support strategy for patients with cardiogenic shock complicat...

متن کاملAcquired von Willebrand syndrome in cardiogenic shock patients on mechanical circulatory microaxial pump support
Early use of mechanical circulatory support, e.g. veno-arterial extracorporeal membrane oxygenation (ECMO) or left ventricular unloading by microaxial pump in refractory cardiogenic shock is recommended in current guidelines. Development of acquired von Willebrand Syndrome (AVWS) in patients with left ventricular assist devices (LVADs) and ECMO has been reported. There is an increasing number o...
